Forward Head Posture (Tech Neck)

🩺 Nursing & Health By CampusDown Wiki Editorial Team Last updated
Forward head posture, often called "tech neck," is when the head sits in front of the shoulders and the neck and upper back start to ache. Long hours looking down at phones and laptops are the most common cause.

What is forward head posture?

Forward head posture is when, seen from the side, your ears sit in front of your shoulders instead of over them. It's a posture habit rather than a formal diagnosis, and it's widely nicknamed "tech neck" or "text neck." An adult head weighs about 10–12 pounds, but tilting it forward multiplies the load: in Hansraj's (2014) model, the neck carried about 27 pounds at 15 degrees and about 60 pounds at 60 degrees.

Causes and symptoms

  • Causes: looking down at phones, monitors set too low, long laptop sessions, weak upper-back muscles
  • Symptoms: stiff neck and shoulders, tension headaches, a rounded upper back, and sometimes arm tingling if a disc is involved

In a study by Kim and Koo (2016), adults with forward head posture had more neck and shoulder muscle fatigue and pain the longer they used a smartphone.

How do I know if I have tech neck?

Stand with your heels, hips and shoulders against a wall. If the back of your head doesn't comfortably touch the wall, your head is probably sitting forward. Doctors sometimes also describe a "straightened" neck on X-ray, which means the normal C-shaped curve has flattened; the two often go together, but they aren't the same thing.

Exercises and daily habits

  • Chin tucks: glide your chin straight back (think "double chin"), hold 5 seconds, repeat 10 times
  • Doorway chest stretch: forearms on the door frame, lean forward gently
  • Upper-back strengthening: squeeze your shoulder blades together

In a 10-week randomized trial, Harman and colleagues (2005) found that a strengthening and stretching program improved forward head posture. Raise your screen to eye level and look up every 20–30 minutes. Long hours bending the wrists can also lead to carpal tunnel syndrome, and long screen time to computer vision syndrome.

FAQ

Can exercise alone fix tech neck?
If it's a posture habit, exercise plus a better setup often helps. Arm numbness or weakness is a reason to get checked for a disc problem.
What kind of pillow is best?
One that supports the curve of your neck and keeps your face pointing straight up when you lie on your back.
